Pre-Employment Forms
IMPORTANT: Employees MAY NOT begin working until the criminal background check and sex offender registry check have been completed. The supervisor must receive notification from HR of the completion of both checks.
New & Returning Employees
For new employees and returning/retiree employees if separated from ¼ø»ÆÊ¦app for at least one year. The following pre-employment forms must be completed and approved by Human Resources:
- Authorization/Consent & Release for Background Check (P-61B)
- or . A Data Record is not required for Non-Credit Adjunct Instructors.
- Course Approval for Instructors (P-24) (adjunct instructors only)
- Employee application and transcripts (full time employees and adjunct instructors only)
